Children’s Healthcare Assistance Guidelines

Project Wellness Foundation, Inc. provides primary healthcare assistance funds for children of families who are uninsured and who are without the financial resources to afford medical care.

To be eligible you must:

• Have no public or private health insurance
• Meet income guidelines (income less than Federal Poverty Level)
• Be a citizen of the United States
• Live in the area the program is offered
• Provide photo identification
• Provide income verification upon request
• Child must be under 16 years and must be accompanied by a parent or legal guardian

Financial Eligibility Criteria for Children’s Healthcare Program

This program will provide funds (to a maximum of $200) for healthcare services, free of charge, to eligible individuals with a total household income under the Federal Poverty Level.  For 2008, we will use 2007 income guidelines.
